Consent & Release Agreement

A completed UACR Consent & Release Agreement is required by all team participants (mentors, students, camp attendees, etc.) as a condition of being a team member and/or participating in team activities.  Please note that if the Participant is a minor, then his/her parent or legal guardian must also sign. 

This form is now signed electronically via MIMS. When students join the team they or their parents will be assigned a task to sign the Consent and Release form. Each year students must complete a new form.
